Longevity and Healthy Aging Physical Therapy in Chardon, OH
You want to be strong at 80. It starts now.
You want to hike, golf, travel, and get down on the floor with your grandkids for decades to come. But you've noticed you're a little weaker, a little stiffer, and a little less steady every year. You're not hurt. You just don't want to slow down.
You deserve a plan to stay strong, capable, and independent for life.

We know how frustrating this is
Most of our clients come to us after other treatments didn't work. We know how discouraging it is to hurt, feel stuck, and not know who to trust. That's why the Doctors of Physical Therapy at Chardon Performance Therapy start by listening, until we understand your story and your goals.
We've helped thousands of active people regain confidence, strength, and control, even after other treatments didn't work.
What we help with
Building and keeping muscle as you age
Strength, power, and balance testing
Aches that keep you from training
Golf, pickleball, hiking, and travel goals
Staying active after an injury or surgery
Ongoing check-ins to keep you on track
How it works
1. Share your story. Your initial assessment is a 75-minute, one-on-one visit with a Doctor of Physical Therapy. We listen to your goals, your history, and what you've already tried, so we understand what truly matters to you.
2. Get a comprehensive care plan. We measure your strength, power, and balance with VALD force plates and dynamometry, so you know exactly where you stand.
3. Work with an accountable guide. Your follow-up sessions are a full hour, one-on-one. We build a plan around your goals and retest, so your progress shows up in real numbers.
Our three-phase approach
1. Pain management. We take care of the aches that keep you from training.
2. We move you. We restore the mobility and movement you need for the life you want.
3. You move you. You build strength, power, and balance that last, and keep it with regular check-ins.
What the research says
National guidelines recommend muscle-strengthening activity at least 2 days a week for all adults, and balance training for older adults (U.S. Physical Activity Guidelines).
Exercise programs with balance and functional training reduce falls in older adults by about 23% (Cochrane, 2019).
What our clients say
"My knee is 100% now and I am continuing in the Life Performance Therapy group. This has been so much fun! Jack is the bomb! He keeps us on our toes and moving like athletes!"
Tracy J. ★★★★★
"My back and knees are much better with the work David has done for me. I'm 64 and in great shape because of him… he helped me improve my overall driver speed by 10 mph or 30 yards."
Rich K. ★★★★★
"He took the time to understand my unique goals, evaluate the source of pain and inflexibility, and develop a program specific to my needs. I am confident I will be a better and healthier 'mover' as a result."

Frequently asked questions
Do I need a doctor's referral for physical therapy in Ohio?
No. Ohio law lets you see a physical therapist directly, without a referral.
How long are the sessions?
Your initial assessment is 75 minutes. Follow-up sessions are 60 minutes, always one-on-one with a Doctor of Physical Therapy.
Do you take insurance?
We're out of network with all insurance companies. We accept HSA and FSA cards, and many clients are reimbursed through their out-of-network benefits. We'll walk you through your options on a quick call.
Do I need to be injured to come in?
No. Many of our clients come to us to get stronger and stay active, not because they're hurt.
Do you offer group programs?
Yes. Ask us about our Life Performance Method, a longer-term program for staying strong and active.
Is it too late to start strength training?
Never. Strength improves at any age.
When should I see a doctor instead?
See a physician for chest pain, shortness of breath, or dizziness with exercise, or any new, unexplained symptoms.
